REFORGE’s Coordinated Veteran Care (CVC) program provides team-based care where the patient, their GP and nurse coordinator work together to achieve patient-centred healthcare goals. This program aims to empower and support our veterans in managing their ongoing healthcare, available to Gold Card and eligible White Card holders with a mental health diagnosis.
The CVC program is a great opportunity to partner with your complete healthcare team to achieve your optimal health and wellbeing goals.
Please speak to your GP or click below if you are interested in participating in this program and one of our nurses will be in contact with you to discuss the program in more detail and organise your commencement date.
What to expect from the cvc program
- access to a dedicated nurse, GP, and holistic healthcare team
- assistance in mapping out your health and personal goals to stay on track
- connection with a knowledgeable allied health team
- engagement with community networks and support services for vocational and educational needs
- availability of health check services, ECG, annual health assessments, heart health, cancer screening and preventative medicine.
For optimal scheduling, we recommend booking appointments three months in advance, ensuring greater flexibility in selecting your preferred doctor.
